Natural Supports
We believe connection is vital.
We work hard to ensure family visits and meaningful connections continue, supporting the everyday relationships that provide emotional, social, and practical support. These connections foster inclusion and independence, and maintaining them is always our top priority.
Health & Wellness
We coordinate annual healthcare requirements to promote overall wellbeing and independence. This includes lab work, medication administration, independence assessments, dental appointments, vision and hearing exams, and risk management planning. Our team is trained to provide safe, informed, and compassionate support.
Supported Living Services
The goal of Residential Habilitation & Support is to empower people to build, strengthen, and maintain the skills they need to live confidently and successfully in their own homes. We meet each person where they are, offering guidance, encouragement, and support tailored to the formal goals established by each participant.
Personal Assistant Care (PAC)
Personal Assistant Care (PAC) is individualized with personal goals designed to assist with accessing community, activities of choice and personal growth as defined by the person. We offer personalized support with daily routines and everyday tasks, allowing people to live with dignity, confidence, and greater independence.
Community Engagement
By focusing on what’s meaningful to each person, we ensure that everyone has the opportunity to participate, engage, and thrive—building confidence, friendships, and a true sense of belonging. Creating unique schedules helps each person connect with the community in ways that matter most to them. From local events to personal hobbies and social activities, we tailor experiences to match personal interests and goals.
Social Connections
We go the extra mile—offering more than basic services with meaningful opportunities for connection and belonging. We genuinely work to enhance social relationships through special events and shared experiences, such as holiday celebrations, Christmas parties, family reunions, Memorial Day and Labor Day cookouts, and weekly summer pool gatherings.
Day Program
MONDAY - FRIDAY | 8 AM - 4 PM
Our Day Program is designed to increase independent living skills.
We offer facility based programs as well as a strong community-based presence.
Day habilitation, which includes community-based habilitation and facility-based habilitation, are services specified in the Person-Centered Individualized Support Plan (PCISP) and support learning and assistance in the areas of: self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Day habilitation activities are intended to build relationships and natural supports.

Check writing, bill paying, checkbook skills, budgeting, shopping, coupons, etc.

Physical fitness classes & education, nutrition, health management and monitoring along with community safety.

Classes on Constitutional Rights,
personal rights, freedom of expression/choice along with advocacy.

Kitchen safety & skills, following recipes, measurements, new tools and equipment.
